Case Manager – Children’s Long Term Support Waiver

Full time position available for Case Manager at the Family Support and Resource Center to work with families who have a child with a significant disability. Responsibilities include advocacy, problem solving, information and referral, short term and long range planning, and management of various funding sources on behalf of children, as well as other tasks related to case management.

Qualifications include Master’s degree in social work, special education or related field, plus at least two years experience working with families who have children with disabilities. Bachelor’s degree acceptable with related experience.  Must have excellent communication, team building, organizational and advocacy skills, and knowledge of Dane County resources. Spanish or Hmong language fluency a plus. Car and valid driver’s license required.
