Full Time CLTS Case Manager
We are seeking a dedicated and compassionate Children’s Long-Term Support (CLTS) Case Manager to join our team. The ideal candidate will have a solid understanding of disabilities and a strong background in behavioral health. This role involves working closely with youth and their families to assess their needs, develop personalized service plans, and provide ongoing support to help them achieve their goals and improve their overall well-being. The Case Manager will play a crucial role in connecting clients with resources, advocating for their needs, and ensuring they receive the appropriate services.
QUALIFICATIONS:
- Bachelor’s degree in Social Work, special education, or related field, plus two years’ experience working with families who have a child with a significant disability. Master’s degree preferred.
- Excellent communication skills, both verbal and written, with the ability to build rapport with clients effectively.
- Ability to advocate effectively on behalf of clients, ensuring their rights and needs are recognized and addressed.
- Experience working with diverse populations and understanding of cultural competency issues.
- Competence to work independently and as part of a multidisciplinary team, with strong organizational skills.
- Knowledge of Dane County resources.
- Fluency in Spanish or Hmong a plus.
RESPONSIBILITIES:
- Maintain a caseload of children who are eligible for the CLTS waiver.
- Conduct comprehensive assessments for CLTS waiver eligibility.
- Develop, review, and revise individualized service plans to identify child’s needs, including:
- advocacy, planning and problem solving with families,
- helping to coordinate resources,
- information and referral,
- assisting families in understanding and working within funding requirements
- Maintain accurate documentation of client interactions and service plans in compliance with agency and county and state policies.
- Provide emotional support with emphasis on individual assets and positive change.
- Assist families in developing crisis intervention plans.
- Provide and promote information on strengthening community support systems.
- Plan coordination with other agencies in providing services to families.
- Comply with procedures required by the CLTS Waiver, Children’s COP, Medical Assistance, and the fiscal agent mandate.
- Complete county and state forms when necessary.
- Assist agency with plans for program development.
- Maintain active communication with Supervisor/Executive Director.
- Participate on committees and task forces relating to FSRC and CLTS services.
